ORDER DEATH CERTIFICATES

• Death Certificate is $12.00 per copy
• Fetal Certificate is $9.00 per copy

Death certificates can be obtained from several different local and state vital records offices depending on when and where the death took place.

The Los Angeles County Vital Records Office Death Section is now able to provide certified copies for deaths occurring in Los Angeles County from 1995 to the current year. Death certificates can also be obtained at any of the Registrar-Recorder/County Clerk's offices.

Pursuant to California Health and Safety Code Section 103526, only specific individuals are permitted to obtain an authorized certified copy of a birth of death record. To order a copy of a certificate by mail for a death that occurred between 1995-present, please fill out two forms:

1. Application for a Death Record
2. Certificate of Acknowledgement (must be notarized if sent by mail)
