When you’re 16 or older, you should start thinking about transitioning to life after foster care. As a young
person in foster care, you are in charge of your future -- it can be exciting and scary at the same time! You’ll need two things: a plan
and some friends.
You might feel nervous about what lies ahead but, with some preparation, you’ll be able to make good decisions and live freely.
Develop a transition plan that is individual to your needs. The sooner you can start working on it, the sooner you can take
charge and make your dreams of independence come true.
Everyone can use some help from a friend! Having a committed and reliable adult you can trust will can help you to transition
smoothly from foster care to independence. Talk to your Children's Social Worker (CSW), attorney, foster parent, probation officer,
ILP coordinator, doctor, and other important people about your Transitional Independent Living Plan (TILP).
