Information Sheet on How to Obtain a Certificate of Still Birth
General Information
- Only a parent can obtain a Certified Copy of Still Birth.
- The Los Angeles County Department of Public Health, Vital Records Office, upon request of the parent, will issue a certified copy of a Certificate of Still Birth. An application and fee must be submitted at the time of the request.
- Applications must be filed IN-PERSON at the Vital Records Office (313 N. Figueroa Street, Lobby-1). The form is available at the counter of the Vital Records Office.
- The Certificate of Still Birth does not replace the Certificate of Fetal Death.
Time Frame
- The Los Angeles County Office of Vital Records archives fetal death records for 1 year from the date of event and only processes requests for Certificate of Still birth for one year from date of event.
- Requests for a Certificate of Still Birth greater than one year from date of event must be sent to the California Department of Public Health Office of Vital Records.
Fees
- Non-refundable fee of $20.00 per copy of a Certificate of Still Birth is charged (California Health and Safety Code 103040).
- Fees can be paid by cash, check**(must be pre-printed with name and address), credit/ATM, or money order to the County of Los Angeles, Dept of Public Health at the time of submitting the application.
- If obtaining a Certificate of Still Birth from the California Department of Public Health Office of Vital Records, please directly contact the State's Office of Vital Records.
